Checking into the hospital can be overwhelming. To help with this process, we've provided electronic versions of our consent forms that can be completed ahead of time, as well as a list of items that should be brought to the hospital. Depending on why you are visiting Rose, your doctor may provide you with additional instructions about what to bring with you.

What to Bring to the Hospital:

Please bring the following items with you:

  • Copy of Advance Directive (DNR/DNI)) (if applicable)
  • Copy of Medical Durable Power of Attorney, if applicable
  • Insurance card
  • Medication list (drug name, dose, times taken)
  • Papers that your physician has provided you
  • Photo identification (license, passport, military ID, etc.)

Please do not bring:

  • Valuables (the hospital is not liable for lost or stolen items)
  • Pillows, blankets, mattresses (Denver Fire Code requires the hospital to ensure patient belongings are fire resistant)
